PLEASE READ: About Billing with The Lactation Network
I participate with The Lactation Network (TLN). Use the steps below to check your eligibility for my insurance-covered care through The Lactation Network if you have the following insurance plans: UnitedHealthcare PPO** and HMO Follow the steps below to check your coverage BEFORE scheduling a visit.

I also charge a $75.00 admin fee for initial visits only, which is NOT covered by insurance. This is not “balance billing” but covers non-clinical, non-billable services. There is NO admin fee for follow up visits. If you have questions about TLN billing, email me at hello@lactationally.com.

BEFORE scheduling your first visit please complete these steps:

1. FIRST, use this link to check your insurance eligibility with TLN.
2. Complete above form in full. (Choosing “Emergent” or “Urgent” is recommended for fastest response.)
3. Wait for confirmation via email/text. (This can take up to 24+ hours).
4. Return to this page to schedule visit when you receive confirmation of insurance coverage.
